Request Transcripts & School Records
Individuals who need transcripts or other school records from New Vision Christian Academy may submit a request by fax, email, or mail.
How to Submit a Request
Please Include the Following Information
- Your maiden name or the name used during your school years.
- Year of graduation or year of withdrawal.
- Name of the last school attended.
- Date of birth.
- State the exact records you are requesting.
- If requesting a transcript for college, include the college's complete address.
- Your signature.
- Identification and current address.
Official College Transcripts
Official transcript copies requested for college purposes are sent directly to the college. Please include the college's complete address with your request.
